Heathy Women Healthy Families

The Children Home Society of NJ received a grant from the NJ Department of Health, for a program called Healthy Women, Healthy Families. Under this new program CHS will be managing Central Intake for Monmouth & Ocean Counties. Also under this program the role of Community Health Workers, has been created. Community Health Workers (CHW) work to promote women’s health regardless of her pregnancy plans, across the reproductive lifespan.

How do I sign up for Central Intake?

Central Intake for the most part, will receive referrals from most OBGYN offices, social service offices etc. But you can also call us directly.

What can a Community Health Worker(CHW) do for me?

CHW’s act as a “bridge” connecting women and their families to programs, agencies, or doctors who can provide health services, screening, and interventions to help improve health outcomes. This is done in a language women can understand from knowledgeable workers who are familiar with their community. CHWs are a one on one peer support for women and their families. The CHW will be providing on-going follow –up and assessment of your needs. So, if you are a woman between the ages of 15 to 44 and need assistance with housing, employment, food, community information, and educational programming, the CHW can help you! If you are between the ages of 15-44 years old and live in Monmouth or Ocean Counties, you can calls us, we can assist you!

What is a Home Visiting Program?

Home visiting programs provide free ongoing health and parenting information, parent/family support, and links to essential health and social services during pregnancy, infancy, and early childhood. HV services are free of charge and include frequent and regular visits to families from pregnancy until the child is age two or three (eligibility varies by model). Central Intake will work with programs in the area to determine program eligibility.


Central Intake

Central Intake connects women, children, and families to community resources and services that address their specific needs. Central Intake will also refer women to Home Visitation Programs in both counties.
